Changing Legal Documents

Changing a legal name and changing gender markers can be done on eHealth Saskatchewan’s website. Both processes involve completing forms, having them signed by a Commissioner for Oaths, Notary Public, or Solicitor, and requesting updated versions of a birth certificate and/or health card. Most of these services require a fee.

TransSask provides the form for a legal name change on our website, as it is ordinarily available only by contacting eHealth directly. We make no guarantee that this form is up to date or that printed copies of it will be accepted.
